I have read, understood and accurately completed this questionnaire. I can confirm I am voluntarily engaging in an acceptable level of exercise, and my participation involves a risk of injury.

If I answered Yes to one of the questions above, I have sought medical advice and my GP has agreed that I may exercise.

I understand in The Fitness Centre there are live CCTV cameras and this will be stored securely and following GDPR rules. 

I give permission for JDD Fitness to post photos and videos on their Website and Social Media Platforms.

erms & Conditions for Group Sessions and Online Coaching Packages

By enrolling in a group training session or online coaching program provided by Joshua Drake of JDD Fitness, you acknowledge and agree to the following:

1.⁠ ⁠Medical Disclaimer

    •    You understand that Joshua Drake is not a medical doctor or registered dietitian. Any nutritional, exercise, or lifestyle recommendations made are for informational and educational purposes only, and do not replace medical advice from a licensed healthcare professional.

    •    You agree to consult your doctor or a qualified healthcare provider before starting any new fitness or nutrition program, especially if you have a known medical condition, injury, or are undergoing medical treatment.

2.⁠ ⁠Non-Prescriptive Guidance

    •    All recommendations, including but not limited to calorie intake, macronutrient targets, training volumes, exercise selection, and lifestyle changes, are provided as non-prescriptive suggestions based on the information you have provided.

    •    These suggestions are not intended to be interpreted as medical, therapeutic, or clinical prescriptions. You are responsible for determining your personal suitability for these recommendations.

3.⁠ ⁠Disclosure of Medical Information

    •    You confirm that you have provided full and accurate disclosure of any relevant medical conditions, injuries, allergies, or limitations that may affect your ability to safely participate in training or follow the guidance provided.

    •    You agree to inform Joshua Drake immediately of any changes in your health or medical status that may impact your participation.

4.⁠ ⁠Program Commitment

    •    By signing up for this program, you agree to a minimum commitment of 90 days. This allows for the proper implementation of strategies, habit-building, and noticeable progress. Early cancellation of the program is not permitted unless under exceptional circumstances, at the discretion of Joshua Drake.

5.⁠ ⁠Results and Responsibility

    •    You understand that results are not guaranteed. Progress depends on numerous factors including, but not limited to, your individual effort, consistency, adherence to the program, and lifestyle outside of training.

    •    You accept that any recommendations provided are designed to guide you toward mutually agreed goals, and it is your responsibility to follow the plan to the best of your ability.

    •    Refunds will not be issued based on dissatisfaction with results or perceived lack of progress, provided the service has been delivered as agreed.

6.⁠ ⁠Assumption of Risk

    •    You voluntarily assume all risks associated with participation in physical activity or dietary changes advised by Joshua Drake / JDD Fitness. This includes, but is not limited to, the risk of injury, illness, or adverse effects.

    •    You acknowledge that participation is at your own risk and that you are solely responsible for listening to your body and modifying any exercises or nutrition advice as needed.

7.⁠ ⁠Communication and Feedback

    •    Should you experience any discomfort, health concerns, or adverse effects as a result of the program, you agree to communicate this to Joshua Drake as soon as possible.

    •    Failure to report such issues absolves Joshua Drake of any responsibility for negative outcomes resulting from lack of communication.

8.⁠ ⁠Conduct and Participation

    •    You agree to participate in a respectful, punctual, and responsible manner. Inappropriate conduct, misuse of services, or repeated non-engagement may result in the termination of your program without refund.

If any of the above information should change, I will let my instructor know at the earliest opportunity
